Home health services
After a hospital stay, a surgery, or a change in a long-term condition, many people can recover at home with regular nursing visits. Our nurses come on the schedule the doctor orders. They check how the patient is doing, give the treatments in the care plan, teach the family what to watch for, and report back to the doctor.

- IV therapyIntravenous medications and fluids given at home as ordered.

How it goes
The referral
The doctor, hospital, or family contacts us. We confirm the order and check Medicare or insurance coverage.
The first visit
A nurse comes to the home, does the assessment, and agrees on a plan of care with the doctor.
Regular visits
The nurse visits as often as the plan calls for. Each visit is written up, and the doctor hears about anything that changes.
Discharge
When the goals in the plan are met, we wrap up care and make sure the patient and family know what comes next.
It starts with a doctor's order
Home health care starts with an order from the patient's doctor. If you don't have one yet, call us and we'll explain how to ask for it.
Nurses visit part-time
Medicare covers home health nursing for patients who are homebound and need part-time skilled care. Nurses visit for a set time and then leave; this isn't round-the-clock or live-in care.
What to have ready when you call
Have the patient's medication list and discharge papers handy when you call (818) 717-7181. It speeds up the first visit.
