About us
Federal Home Health Agency sends nurses, therapists, and aides to patients' homes so they can recover in familiar surroundings. Our office is in Porter Ranch, and our team cares for patients across seven counties.
How we work
We work with each patient and their physician to put together a plan of care for that patient's needs, then carry it out at home.
For most of our patients, that means they can stay in familiar surroundings and keep their routine while they recover.
At home
Visits happen where the patient lives, with their own routine and family nearby.
With the doctor
Each plan of care is set with the patient's physician and changes when the patient's needs change.
After the hospital
Care often starts in the first weeks home after a hospital stay or surgery.
