Table of contents
1. What information do we collect?
Personal information you disclose to us
In short: We collect the personal information that you send us through the callback form or the resume form.
Callback requests. When you ask us to call you back through the website, we collect:
- your name (required)
- your phone number (required)
- your email address (optional)
- the county where care is needed
- the best time to call
Resumes. When you send us your resume through our careers page, we collect:
- your name
- your phone number
- your email address
- the kind of work you do (nursing, therapy, home health aide, office, or not sure), picked from a list
- your resume, as a PDF or Word file of up to 4 MB, and whatever it contains, such as your work history, education, and professional licenses
The website does not keep a copy of your details or your resume. Both go straight to our office inbox by email, with the resume attached.
Neither form has a message box. There are no accounts, sign-ups, or payment fields on this website.
Health information
In short: This website is not used for patient records or medical information.
Neither form asks about anyone’s health, and neither has a message box where health details could be typed. Apart from your name, phone number, and email, the callback form asks only for your county and the best time to call. We go over the patient’s needs, medications, and insurance by phone when we call you back. Please don’t send medical details to us by email either. If you do, you can ask us to delete them (see how to review, update, or delete your data).
If you send us a resume, please leave health information and your Social Security number out of it. The resume form says the same.
If you are referring a patient, use the callback form only to ask for a callback. Send the referral itself by phone or fax, as our referral page explains.
If you are or become our patient, the health information we keep about you is protected under the Health Insurance Portability and Accountability Act (HIPAA). How we use and share it is described in our separate Notice of Privacy Practices, not in this website notice. You can also ask our office for a paper copy by calling (818) 717-7181 or emailing info@federalhha.com.
Sensitive information. We do not ask for sensitive personal information through this website, including in the resume form.
All personal information that you provide to us must be true, complete, and accurate, and you must notify us of any changes to such personal information.
Information automatically collected
In short: Our hosting provider keeps standard server logs, which include your Internet Protocol (IP) address and basic browser and device characteristics.
Like any web host, Vercel, the company that serves this website, automatically records limited technical information when you visit: your IP address, browser and device characteristics, and the pages you request. We do not use this information to identify you. It is needed to keep the website secure and running.
We also use IP addresses to limit how many callback requests and job applications can come from one address in a short time. For that, the address is held only in the memory of the server handling the request. It is not saved to a database or added to your request.
This website does not set cookies and does not run analytics trackers or advertising pixels.
2. How do we process your information?
In short: We process your information to call you back, to consider your job application, to send a one-time confirmation that your request or application arrived, to keep the website secure, and to comply with law.
We process your personal information for the following reasons:
- To respond to your request. The callback form goes to our office email so we can call you back. If you gave an email address, we may also reply by email.
- To consider your resume.Your resume and contact details go to our office email. We review them and contact you by phone or email when there’s a role that fits.
- To confirm we received it. When you send a resume, we send one email to the address you gave confirming that it arrived. For a callback request, we send one only if you gave an email address, and it also asks you not to reply with medical information.
- To protect the website. We use technical information such as IP addresses to prevent abuse of the forms and keep the website secure. Both forms also have a hidden field that people never see. Automated spam programs fill it in, and those submissions are discarded.
- To comply with our legal obligations. We may process your information to comply with applicable law, respond to legal requests, and exercise, establish, or defend our legal rights.
We do not use your information for marketing. The only email this website sends you is the one-time confirmation described above.
4. What is our stance on third-party websites?
In short: We are not responsible for the safety of any information that you share with third parties that we link to from this website.
This website links to a few third-party websites that are not affiliated with us: Google Maps (for directions to our office), Medicare.gov, the California Department of Health Care Services (Medi-Cal), the California Department of Aging, the U.S. Department of Veterans Affairs, the U.S. Department of Health and Human Services (HHS.gov and its Office for Civil Rights complaint portal, linked from our patient notices), and the website of the company that built this site. We do not make any guarantee regarding those third parties, and we will not be liable for any loss or damage caused by your use of their websites or services. Any data collected by third parties is not covered by this privacy notice and is governed by their own privacy policies.
6. How long do we keep your information?
In short: Your request or application stays in our office email like any other business correspondence, and no longer than necessary.
We keep your personal information only as long as necessary for the purposes set out in this privacy notice, unless a longer retention period is required or permitted by law, for example when information becomes part of a patient record that health care and Medicare rules require us to keep.
Resumes stay in our office inbox for as long as we need them to consider you for a role, and for as long as employment record-keeping laws require us to keep job applications. The website itself does not store them.
If you would like your request or application deleted, ask (see how to review, update, or delete your data) and we will remove it unless we are required to keep it.
Server logs are retained by our hosting provider for a limited period as part of its normal security and reliability practices.
7. How do we keep your information safe?
In short: We protect your personal information with organizational and technical security measures.
We have implemented appropriate and reasonable technical and organizational security measures designed to protect the security of any personal information we process, including serving this website over an encrypted (HTTPS) connection. However, despite our safeguards, no electronic transmission over the internet or information storage technology can be guaranteed to be 100% secure, so we cannot promise that unauthorized third parties will never be able to defeat our security. Transmission of personal information to and from the website is at your own risk.
8. Do we collect information from minors?
In short: We do not knowingly collect data from or market to children under 18 years of age.
We do not knowingly collect, solicit data from, or market to children under 18 years of age. By using the website, you represent that you are at least 18 or that you are the parent or guardian of such a minor and consent to such minor dependent’s use of the website. If we learn that personal information from users less than 18 years of age has been collected, we will take reasonable measures to promptly delete such data from our records. If you become aware of any data we may have collected from children under age 18, email us at info@federalhha.com.
9. What are your privacy rights?
In short: You may review, change, or request deletion of your information at any time, depending on your country or state of residence.
Withdrawing your consent: If we are relying on your consent to process your personal information, you have the right to withdraw your consent at any time by contacting us using the details in how can you contact us about this notice? However, this will not affect the lawfulness of the processing before its withdrawal.
Marketing communications: We do not send marketing or promotional email, so there is nothing to unsubscribe from. The only email this website sends is the one-time confirmation that your request or application arrived.
Your information
If you would at any time like to review, change, or delete the information we hold about you, email us at info@federalhha.com. Upon your deletion request, we will delete your information from our records. However, we may retain some information where required to prevent fraud, assist with any investigations, enforce our legal terms, or comply with applicable legal requirements, for example a patient record that health care and Medicare rules require us to keep, or a resume that employment record-keeping laws require us to keep.
You can also skip the callback form entirely and call our office at (818) 717-7181. The form is never required to reach us.
10. Controls for do-not-track features
Most web browsers and some mobile operating systems include a Do-Not-Track ("DNT") feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. No uniform technology standard for recognizing and implementing DNT signals has been finalized, and we do not respond to them. In practice this makes no difference on this website: we do not track visitors, with or without a DNT signal. If a standard we must follow is adopted in the future, we will describe it in a revised version of this privacy notice.
California law (Business and Professions Code section 22575) asks websites that track visitors across other websites to say how they respond to DNT signals. This website does not track visitors across other websites, or on this one.
11. Do United States residents have specific privacy rights?
In short: California and a growing number of other states have consumer privacy laws. Depending on the state where you live, you may have the right to request access to and receive details about the personal information we maintain about you and how we have processed it, correct inaccuracies, get a copy of, or delete your personal information. These rights may be limited in some circumstances by applicable law.
